When applying to the Center, you need to have:
- An electronic referral letter to be consulted by a specialized expert from a pediatrician/family doctor that you have entered into an agreement with or from a specialized expert;
- Child’s birth certificate and passport of one of the child’s parents or legal representatives;
- Record of the child’s development or outpatient’s medical card (form 112/o).
In case of a child’s illness, you can consult a pediatrician, gynaecologist, psychiatrist without an electronic referral letter.
